how do i make them? i need one for my site www.habbonotes.tk

Well since you are hosted by geocities then you cant write a php form handler so I suggest using the thybag service www.thybag.co.uk. Also maybe look at the form tutorial on there for how to constuct text boxes etc ;)

Also he won't be able to do that as he's not on a PHP allowing host "/
