This is mostly from a tutorial on www.good-tutorials.com as is your web hosting web layout if not its all from good-tutorials you just pasted the image.

I like it, but where the Username & Password bit is, i would get rid of the

If you double click on the boxes then in the 'Initial Value' bit type 'Username..' and for the password one 'Password..'. This will then make it inside the box. Just and idea :P
