The best is xhtml & css.

The easiest(my way)

ImageReady or Fireworks.

Image ready and firefoxs isnt you coding your layout, its you pressing a button that makes the app **** out a poor exsuse for a layout that will bairly work.

html, xhtml, css, images and somtimes a bit of js
