If you look in cpanel then fantastico theres is a bit called polls and surveys. Or you can go to

There loads of sites that you can make a poll on if you search on google :).

a good one is www.internationalvoting.com i use it for my DJ of the week poll. Just copy & paste a bit of HTML in2 ur site. Simple!
