Need Job Application Script

Hello, I need a Job Application script, where people who are applying can fill it all in, and add a sound clip. Then it will send it all to a Email. Can some one please help. + Rep if u do. (I Give any 1 rep who helps me :D )

www.thybag.co.uk has one but it doesnt have the sound upload bit. You could easily add it though if you search that bit on google.

You'll have to get them to upload that sound clip onto somewhere and they have to include the URL where it is located I guess:o I'll contact you if I find anything I doubt I will though. Best of luck to you anyway;)
