all that happend is that npc have been given the capes to show that there a master in that skill

healers - 99 hp
melee trainer - 99 def
Captain Izzy - 99 agli
guy out side fishing guild - 99 fishing

or it just the people who give you the capes

They are the givers of capes of Achievement (99 skill capes)
Audbury gives the 99RC cape
Healer in duel arena gives 99 HP
Melee trainer gives 99 deffence
Izzy gives 99 agility
Fishing Guild doorman gives 99 fishing.
Just think about it a little bit. Say the salesman for the mystic robes has one on, He must be the 99 magic cape dsitributor :)
