Strangely enough there is a request and timetable script in GIANT Developments collection... it is the one created by bornhj.

It is very easy to use and understand and best of all is the fact that this script does had require MySQL - all you do is upload and start using straight away with no hassle. Recommend this script to anyone with a basic understanding of PHP and could fix basic errors when they come across them.
