Frontpage is only really for HTML. Forms are done in PHP, either one where it emails you the information (Like you want) or puts the info into a database.

Err I'm not to good with PHP, it takes a while to make a good form :P

But if you research PHP sites, you'll find one easily.

you wouldn't have to use a db for that just use an uploader.
