May mean he has torn a blood vessels inside his privates. These can take a few days to heal - even though this may be the case he should still see a doctor as it can cause clotting inside.

Sometimes blood in your semen is a sign of an STD or Prostate Cancer which is obviously very serious. However, its likely that this isnt the case. A doctor would be the best person to see, or a Urologist. Make sure that everythings okay. They will run some tests and if they cant see anything wrong tell them about the "Getting hit in the knackers" story. Im sure that has something to do with it.
