on how you make your picture proper cartoony..
If you don't understand ill try and get a picture..( which will properly be scribbled)
Do you mean turn a normal photo into a drawing?
If so, there's an effect in Photoshop.

If you want to make an image from scratch like that you need ilustrator

It looks like it, just went over the hate and face with a paint brush tool.
No effects are as crap as that.
