On the last mission, did you choose Deal or Revenge?
Deal will have you try and collect the money from the deal with Phil, but you get screwed over and have to fight to retrieve the money. You'll get 250K from this, and then wind up at Roman's wedding, although Kate will be angry with you for going against your emotions for cash, and will not turn up at the event. Dimitri will turn up with his men and do a drive-by on Roman, killing him. You then go on a vengeful rampage with Lil' Jacob at an old casino, revealing that Dimitry has teamed up with Jimmy Pegorino. D excecutes Pegorino. You eventually kill Dimitri on Happiness Island in an excellent moment of moral irony. Game over.
If you select Revenge, you will go after Dimitri on his both whilst he is supervising the loading of a heroin shipment. You execute Dimitri on the same boat that you arrived at Liberty City in. Kate is pleased for you that you didn't emotionally sell out for cash and then she goes with you to Roman's wedding. In a rage, Nico, Lil' Jacob and Roman go after Jimmy at the old casino, and eventually you kill him on Happiness Island. Game over.
