Forms with upload boxes?

Does anyone know how I can do this?
I've tried a php form generator (which can be found at www.birminghambands.com/formgen )
But it doesn't work for me.
Could someone help me, by either telling me what's wrong with the form generator on my site, or suggesting another one.
