if you write to the national data base thing (there should be an address on the site)
or search national insurence number into google and it should come up with a site
email them
they'll have to do a set of questions
answer them and you should recieve a new one
if not ill look at my letter and if it says anything about losing it and how to reclaim it ill pm you :D

What do I do if I have lost my National Insurance number card?
Report it to your nearest DWP office, Social Security office in Northern Ireland or HM Revenue & Customs office. If you want a replacement card you will need to complete an application form. Remember that you are only allowed one replacement.
Note it says you are only allowed one replacement, that means for your whole lifetime, so before you get a new one I would really have another good look for it!

I lost mine awhile back, but what they did is wrote my number down on some official letter thing rather than giving me a new card, so I can still get another card.
Found it a few months later though.
