i would mention the flirting with guys. i wouldn't just go dump her straight away, just tell her you don't like it and if she gets defensive about it just leave it for a while and if she doesn't stop, finish it. just say you warned her you didn't like it. idk.

yeh, just say that your sorry but that you cant cope with her not trusting you at all, even thought the only one who is flirting with other people is her. Then she may try and say she will change etc... if she doesnt want to break up. But it seems like its not just the flirting thing thats annoying you an you havnt said anything posotive about her so i would say be strong and just tell her its best you both move on as friends.. blah blah basically say whatever feels right at the time, you cant have like note cards and read what we say! breaking up with people is really hard especially if they get upset, so good luck. x
