The gunman is reported to have walked up to the victim and said "Hello" before cocking his revolver and shooting him in the back of the head.
He told staff to call police before sitting down on the café floor and waiting for officers to arrest him, witnesses were quoted as saying.
The victim, believed to be in his 30s, was pronounced dead at the scene in the D A Sandwich Cafe in Court Road, Mottingham.
Armed police raced there after a member of the public dialled 999 shortly before 3.45pm yesterday.
They arrested a man, believed to be in his 40s, inside the cafe about 25 minutes later and recovered a revolver.
Witnesses said the victim had walked in with a friend and ordered two cups of coffee.
The pair were sitting at a table when the shooting happened.
The cafe owner, 38, who refused to be named, had popped out to buy some milk, and returned to see the aftermath.
He was quoted as saying: "When I came back I was faced with a man with a gun in his hand."
A Metropolitan Police spokesman said the dead man has not yet been formally identified.
He added: "We attended with London Ambulance Service staff and the air ambulance.
"They discovered a man inside the premises who was in possession of a weapon, and a victim suffering a gunshot wound.
"He was pronounced dead at the scene. The victim is believed to be in his 30s.
"The arrested man is in his 40s and is currently in custody at a south London police station."
