you could lay it out like

NAME
(home number, mobile number, email)

PROFILE:
(eg, 'enjoyable to work with' and all that, big yourself up)

WORK EXPERIENCE
(list where you did work exp, what you did etc)

INTERESTS
(what you like doing, obviously)

EDUCATION
(write down your school name, GCSEs taken and what you're doing now, eg college/sixth form)

then you could write your address at the bottom

Write how flexible (what hours you can work) you are somewhere.

Put all your formal qualifications on? Or what you are studying to take if you haven't done your GCSEs yet. What you do outside of school. Which school you go to and have been to etc

I did exactly the same as Cocaine.
Well, my mum just made me one and I filled in the boxes. But it looked like that... :l
Don't worry, once you get going it's easy enough.

do you include things that you do that have nothing to do with work?

like sports you play?

would anyone be able to give me an example of one?

Like just one example for each catagory or just a more detailed description?

ive just this hour had to redo my c.v as it needed updating :P

one thing ive picked up on is to make sure when u give any training / work experaince is to put the most up to date one first then go down from most recent to oldist.

refferances can be gained from anywhere really from training programs to colleges to work experainces or any volentering work anything really even from your school if needs be its just basicly someone who has knowen you and what u are like if you are punctual or not things like that so anyone u have been in a work enviorment is a good referance from.
