You have to appologise basically, and wait atleast 3 months on a new account without any bans or anything to prove you can follow the rules without breaking them.

Don't bet on it, staff are harsh and won't usually unban, does depend alot on what your ban was for though.

don't bother appealing until at least 3 months have gone if it's for scamming or scripting. don't appeal at all if you've nicked someones furniz unless it actually wasn't you that did it (no, 'my bruva woz on ma acount' wont work).
