Anyone else having this problem?

I'm going through the campaign on veteran for trophies and for some reason, some levels seem to change from veteren to recruit without telling me (like when I leave and go back). I went through like half a level wondering why I didn't die.

I've played through recruit then veteran afterwards and it didnt happen to me, so idk maybe it was a one time glitch.

happened twice :/ And I'm only like half way through.
