I need help in making a email form.

Please create me or tell me how to create a form and it needs to include:

I would like (if you can style) it to look habboish.

To do so you would need a form.

<form blah>

and userinputs

<input blah>
<textarea blah>

etc

and then you would need to post them to a server side page (PHP :D)
