The way most people who are serious about stretching their ears do it is to go a size a month. That lets the ear heal up and get nice and fatty again for more stretching. It depends on whether you're worried about blowing your ears out, which will prevent you from stretching them to big again.

Don't do it. My friend stretched her ear to 20mm and now she seriously regrets it. Your ear gets all scabby, can get infected, will be very painful AND its not attractive at all... She's trying to shrink it which is very hard (shes going to have to cut the skin in the hole since its all scabbed over and won't heal) Not nice at all.
Like Alex said, you can safely go up one size every one to a few months, but it usually depends on how your ears react. You should have little to no pain while stretching and you shouldn't have any blood. If you do, you should stop and give you ears a few days or even weeks to recover. It means that your ears aren't ready to go up to that size. The most successful and pain free way is using tapers. I think I'm at a 4ga and I've used tapers, there was a little pain, but mostly just discomfort which went away quite quickly. You can use oils etc. to make the skin more maliable and make the stretch more comfortable, quicker and easier. And make sure everything is sterile to reduce the possibility of infection (not just the taper, but with your hands as well).
I found all this out with just a simple search on Google btw :P, it can give a lot more advice than I can.
My mates got a 12mm stretch in atm, he leaves it in usually for around 2 weeks.
He hasn't had any problems except it looks really *REMOVED ugly.
