Install the database mysql.
If you look on first page i said i have set up mysql with the right info.. But the index.php says Choose a username.Quote:

Its asking for help about a specific one.. not which one should I use.
From the screenshot you sent me in that PM tom it looks that you do not have your mysql uploaded.
You need to click on 'Tom' in that dropdown you have. On the right frame click on import and browse to the sql file in the DJ panel directory on your computer.
