ugh on that teen thing, the only problem, heard is one where there is like a skin coloured "ridge" in skin, going from foreskin downwards...

Every day you have to pull the foreskin back as far as possible. At first you do this when your thing is soft. Don’t use force, because then you may cause fissures in the foreskin. So do it consistently, but carefully.

Be aware of the danger of paraphimosis (the foreskin gets stuck behind the glans). If it ever happens to you, seek help immediately.

Few days old this thread, but thought I might be able to help. I had almost the exact same problem (except for the yellow bit), and I didn't want to go to see a doctor (bit shy ^_^) so I just looked it up for ages on the 'net, and like people have been saying, you've got to sort of stretch it a bit. Take it slow, I didn't heed that warning and it kind of... split >.< Painful and there's scarring. Try it in the shower, it's easier to do, but as Jaxxy said, be careful that you don't get stuck!
