Aren't they if a member is on your contact/friend list? I'm sure that's the only reason I've had +s.

I use it as a bookmark. Don't know how useful it is outside of that though tbqh

Quite an old system/feature that remains in vBulletin that doesn't seem to be used by a lot of people.

It's used by me. Don't remove it. I don't want people spamming my VM :(

I remember when it came out haha,
think i used it at first, but haven't seen anyone with a + for a while for mee

I have no intentions to remove and it's like one of the core features of vBulletin anyway.
