How do you put a form on your site s I can't work out how to +rep for help

I have but it isn't appearing on my site :s

Do I upload into ftp or html?

What type of forum is it?

Nvm I got it up but..... I have a request bit and a submitted part but they are different pages do I just make the submit button link to that page? If so what is the html code

Do you have alink to the forum so that we can see?

First its a FORM and the link is www.rkdesigns.shield-hosting.com/request.html and never mind the stuff in the boxes as its still on the same stuff as when my mate gave me it :P

Try searching www.google.co.uk for a button link.

There are a bunch of forums you can use ~ www.phpbb.com, www.vbulletin.com, www.mybb.com, www.invisionpowerboard.com
