Yes. Good find except I can't submit. Filled it in about 3 times and it keeps coming back blank ;s

I think the reason is that you have to put the word "None" in the "other" box which should only be activiated if you click Other option.. Rather rushed poll methinks.. I hope they fix it.

Lol i know i was like wth is going on? i completed it and it said i had missing fields then i realised i had to type "None" in the other boxes? whats the point in that :D

i put N/A in those boxes XD
