Make her something personal. For example, a handwritten letter. Reminisce about the good times and end off with something sweet like "I never thought I could like someone this much" or something less cheesy. Speak honestly.
Spray a little of your cologne on the letter. Buy her a bear, one that is holding a small red cushion heart. Make sure the bear has soft quality fur and doesn't say "I love you" unless you mean it. Hand it to her when you meet. Save the letter for the end of the night.

You could cook her dinner or else if you're not capable of cooking (my boyfriend certainly isn't! :P) you could talk your mom into cooking some stuff or else you could go buy some premade stuff from the grocery store and have a sort of picnic in the dark if you have a park nearby or something of that sort. Go all out - bring a checkered cloth to sit on, and candles to turn it into a romantic dinner for two. You can also bring wine glasses and sparkling cider (or even if you dont have the sparkling cider, you could drink water or whatever from the glasses - it just seems to give it a nice classy edge that she might like). It should be relatively cheap (you'd only need to pay for the candles if you don't already have some at home that you could take, as well as for any food), and then you could also invest in some flowers for her to give her either at the beginning or at the end. And if there isn't a park or outdoor location you could go to, then you can always just use your house - ask your parents to leave you alone and then set up candles on the table, dim the lights, put on some jazzy music or something. Sets the mood quite nicely! ;)
...Or you could just pull the hallmark and write her a letter/give her a bear. If you're not super close with her yet though, a letter might be a bit much.. A bear + a little bit of jewelry might be nice though, like a necklace or something that she could wear to remind herself of you. :)
