You can do it online at www.xbox.com/support or call 0800 587 1102 and do it over the phone. I'd do it by phone and they should explain exactly what you need to do.

Actually, you have to take your hard drive (its the grey thing on the top) off and then send the console. Xbox normally send you a box to put it in, and then an address to follow.
