How do I create a HTML Mail-To form that sends the Users IP with it?

Needs to be like:

From Email Address
Date And Time Sent
Senders IP Address
Subject
_________________

MESSAGE HERE

[nets]

You can't do it with HTML.

You can use my remotly hosted script if you want html only, since it collects the Ip on route
