Website making?

When you make a website do you make the layout into a temeplate in a program e.g dreamweaver, to add to each page or do you code the layout again to add to that page?

Just make the layout. Say you have a middle box where you want the content for page x to be, just simply type some text in file x and do add this to your middle. <?php include("x.html") ?> and it will show.
