Right, how I understand it is that:

When you a type a TLD in (.com/.uk) w/ever, your ISP sends your connection to that TLDs administrator (.com is administered by the Verizon Corporation, the UK one is run neutrally). That organisation has a long list of domains and their respective IPs/DNSs. And then that infomation directs you to the website. The organisations ask for a fee to register from registras, so that the service can carry on. You cannot just make your own TLD because it needs to be approved by the IAAC, who manage the servers that keep some parts of the internet alive.
