Strangely enough there is a request and timetable script in GIANT Developments collection... it is the one created by bornhj.
Request System 2 (Bornhj)
This request system is identical to the original (above), but also requests a very well made schedule system! It is very easy to use and understand and best of all is the fact that this script does had require MySQL - all you do is upload and start using straight away with no hassle. Recommend this script to anyone with a basic understanding of PHP and could fix basic errors when they come across them.
