Looking for a very basic form.

All i'm looking for is a form. Which is very easy ccustomizeable
so 2 files, the form itsself and the action file, I'm able to make forms myself but i'm no good with the action file. I've tried all the ones people have provided but im getting no luck. if you can help thanks.
