Realistically, if this situation were to come up I doubt it'd be a matter of guilt afterwards because you're not in the car thinking "alright, I'm going to kill my friend right now, escape and no one will know". It'd be more of a "LET'S GET THE **** OUT OF HERE" and I'm sure you'd try to help your friend but if time is running out you'll try to live. When you're in a situation like that you can't really think logically, you're just trying to get out. That being said, I'd probably try to help my friend, realize it was hoping and it's their time to go and then save myself. Also, I think it depends on what exactly is happening/how the death is about to go down.
