Only the one. I had two, one on a baby tooth which fell out about 10 years ago
The one I have now was on an adult tooth and I cracked it running face to face into another boy at school, who strangely cracked both his front two teeth :S
I'm bad with my teeth, but the dentist says they're perfect

when i fell off my bike onto my face down a hill (sounds funny but i had to go hospital) i have a biggg scar on my eyebrow line, which thankfully is hidden by my hair. but my dentist said that he would have cried if i had damaged me teeth in any way from that fall lol
