I'm guessing you're focusing on boys at this point. (Not to be sexist or stereotype, but at that age toddler girls aren't really interested in guitars.)
    1. You need an introduction to why you're doing the survey. e.g. I am conducting this survey to investigate the entertainment and learning sectors of children's musical interests.

Could I take a moment of your time to fill out my questionnaire?
    2. Carry out:
    i. Do you have children?
    ii. Would you like children?
    iii. What's your child's age?
iv.Is your child musical in anyway
v.Would you be interested in buying them a guitar specifically designed for toddlers.

Just a few suggestions hope it helped!

    superhappy.
