Go through it with an infiltrator/solider.
Never rush.
Patience.
Do all missions, you'll need the money and upgrades. You want to upgrade everything. This will make life MUCH easier.

You need the right combination, not just "Jack or Samara as biotic" I've seen people choose obvious ones and still have people die, this is what I did.

First part

Tech - Legion
Second Squad - Miranda
Personal - Samara and Mordin

Second Part

Biotic - Samara
Escort - Zaeed
Second Squad - Miranda
Personal - Mordin and Garrus

Final Part

Personal squad - Samara and Mordin

You need everyone loyal and all ship upgrades, that's it, you don't need any personal upgrades at all, with that set up no one dies, Mordin nearly falls but Shepard catches him and Miranda gets shot but survives.

Also some tips for Insanity,

After every single fight, save.

Make excessive use of biotics and tech, primarily Overload, Warp and Reave, when barriers, shields and armour are down use anything like shockwave, push or pull to stop them from being able to shoot.

And when you're on the first planet where you meet Tali for the first time you fight a mech, when you start that part you start against some crates, those crates are your best friend, keep using them for cover, when the mech is next to them aswell move around them, keep them between you and the mech, occasionally it will stop moving, unload Mirandas and Jackobs biotics into it and shoot it as much as you can at those points, you can do that whole fight without getting hit once.

You can but you have to start like as soon as you get on the ship, she's the only female you can romance with if you're female as well.

Garrus/Jacob for squad leader second time
Grunt escort crew back
Mordin + Miranda I took to fight the boss.

As long as everyone is loyal and you've got the upgrades the option above will work. There are multiple variations of how to do this achievement

There is but if you choose one person wrong for your personal team then they might die, which is why I posted mine because it is a certain working combo.
