Happens to me all the time, if you have the disk in there and you accidently tap the XBOX or run into it, or anything really shake the desk, it will scratch it.

Those ones you buy that cleans them, doesn't work hardly at all.

i dont know how so many people get their disks scratched all the games i have work perfectly since day one of getting them
