When you are mining sometimes a rock will smoke, if you continue to mine the rock will explode, bending/breaking your pick axe and hurting you.
Go to the mines north of Falador and go down the ladder. Go straight ahead then right, then it is either the first or second right that has a Dwarf at the end who will fix your axe for a price
