Sonic 2006 had one, unlock the true ending.

I got to it but the achievement stayed locked.

You must give up fast food for atleast a day, achievement unlocked.

100% on Bully: Scholarship Edition. I've done everything possible, 99%. The bleeder is still locked. Ive got apserloutly everything, every other achievement, but no. :\ I completed Cod WaW on Veteran, mind, it take bloody ages.
