Chances of it being real are about 95% as when RS2 was first released as a members beta test you could drop anything, people sold phats for 1gp and gave them away for free so that is most likely real, but then again there is a zamorak kite shield there, which wasn't released until a few months later so it could be private, but taking into consideration that people on RS are idiots it could just be some guy that got bored and dropped everything in the real version

No, you can't. Like I've said before it's just a REALLY old picture - hence the items cost a lot less, hence people dropped them.

Actually you can get NPC's on private server.. BUT the picture displayed isn't private. On private you can do almost anything as Neil said.
