I have seen a stagiere doing the back & front last to a patient....which is not hygienic at all.
I sit down, not standing up. Front to back is most hygienic way (well better then the other way anyways lol)

Do you stick your hand in between your legs and have at it or go in from the sides somehow? Surely you have to hover above the toilet and not really be sat down?
