Feedback like Uplodu

Can someone please direct me to a tutorial on how to get a feedback button like the one on www.uplodu.com. I searched for it a couple of times but didn't get anything. Also it would be helpful if you could help me find one that uses jQuery.

Why not ask the man himself :P

Uplodu uses Uservoice (http://uservoice.com) indeed, you can change the position of the Tab, and it's colour to any HEX Code ;)
