Basically on the last bit says prove that this account is yours. If it still appears in the highscore then i dunno. but if it does not appear in the high score then talk about ur stats.
And at the bottom it usually says like Appeal = Bad, Good, Etc just try provide as much information as you cana nd in the essay bit say that you purchase members with credit card?

Tbh i wouldn't write an essay because they don't really read all that stuff
just like they don't check ban appeals and stuff
