It's different depending on where you go. The one they showed on Penn & Teller goes under your foreskin, lifts it up so it's just the skin stretched over a bit of metal in the shape of a bell then they cut around (hence the word origin) with a scalpel.
Note: Circum (around) -cise (to cut) so cut around.

It is alot cleaner and you are less likly to catch HIV or somthing but you lose sensitivity and once it's gone you cant exactly get it back...
