Whatever it is, it will be harmless. It could be something like the micro-Sim inside, or the battery - assuming the battery isn't bolted down. I'd assume it's slightly loose so it doesn't heat up as much, but this is just guessing
Where abouts is this rattle? You could pin point the sound, look at an image of an open iPhone and discover what it is from that. Google seems to suggest it's either the auto-focus lens, proximity sensor or some sort of bumper.

They're supposedly meant to. Try holding it face down and then tap around where the camera is.

mine doesn't rattle and now i'm PARANOID
EDIT: Oh yeah it does at the top.Mines a 4s.
