It's just a difficulty mode of the single player campaign. Less health, less ammo, little to no GUI and no ability to "see" through walls.

If you're on PS3 and have it fully updated, then you already have it and it's included in the PS4 one from the beginning. I honestly found it easier than survivor mode, but that might be because I've had more practice for grounded mode

Probably. I haven't finished it yet, but when I did it on survivor I got the trophy for that difficulty and below. Otherwise you'd have to complete the game 10 times to get all the completion trophies If you want, you can wait until I've finished it which shouldn't be too long from now and I'll let you know.
