Only happened to me once, my best friend liked this girl called Laura and I asked Laura if she liked him, she said no and that she liked me, so I asked my best friend if it was okay if I got with her and he said it was fine.

I did like this girl as a friend but I thought "if she likes me and we're good friends, might as well give it a go"

Definitely off limits tbh. If they dated them/had a crush on them in the past three years, it's a big nono.

Without stealing the thunder of the thread, what about friends' siblings?

FYI my best friends sister is smoking. Could've done a couple of years back, didn't because I respect boundaries but hell, I'd rather have somebody I know date my sibling than a randomer. Just my view. I think exes are a tricky issue and it depends how the relationship was as a whole.
