This you don't need a script for, you can use forms, for this you'll need frontpage or dreamweaver etc.
I quickly searched google for form tutorials and came up with this

Only people that know php can do that you need to be able to parse html or something its hard to do and only a few sites have done it eg hobbas and mods.
