You might want to state whether the form will be either submitting to a database or whether it will be sent via email.

UNable to eidt anymore but i needf this badly and i can use database or email waht every is easyier

So basically a form with an uploader? If you search for the user 'Excellent' (me) I have both of these, it's just a case of you putting them in together
