PHP File Editing Help

Hey!

Just a quick post.
I have a PHP file I need editing to add some fields, I personally do not know how to do it without messing them up! It should take the right person under 15minutes to do and any help would be greatly appreciated and can pay you for the work with Paypal.

1, Locate file
2, Edit file
3, Add fields
4, Save file
